May not even be from infection, unless diagnosed by the doctor. Could be an enlarged prostate.
Med: possibly something to control the enlargement. Please have him consult a doctor immediately.
If he really hasn’t urinated in four days, he would be in immense pain and possibly a toxic stupor by now. True urethral blockages are very uncommon in humans.
Sounds like he needs a doctor, not some random stranger online.

He really needs to go to the doctor asap or possibly the emergency room. This could mean enlarged prostate or some sort of infection or cancer. Also could be kidney stones or urinary tract infection, which may lead to kidney infections. Get help please.
He must go to his MD ASAP, he could damage his kidney & cause a kidney infection from all the toxins remaining in his body.
Isn’t he in pain?
He needs to go straight to the doc. It could be prostate cancer or enarged prostate. NOTHING to look anywhere else for. Very dangerous. Kidneys might have shut down. so please don’t wait..get him to the ER!!
